We are looking for a Youth Outreach Support Worker to join a well-established Charity in South Wales on a permanent contracted. This role is offering great package and hybrid working.

Youth Outreach Support Worker responsibilities:
- Support people aged 16-24 across Monmouthshire who are affected by homelessness and struggling to find, secure or maintain appropriate accommodation
- Undertake assessments of need in accordance with the support assessment procedure
- Ensure that all appropriate actions are taken to prevent unnecessary tenancy/accommodation breakdown
Youth Outreach Support Worker requirements:
- At least one year experience working with individuals experience homelessness
- Social housing experience
- Full UK driving license
