Nimbuscare in York is seeking an experienced registered nurse to join the Community Frailty Hub. The post holder will supervise a team of nurses and healthcare support workers with support from the clinical lead within the Integrated Neighbourhood Teams, delivering proactive nurse-led Comprehensive Geriatric Assessments at home.

You will also work within the Complex Frailty INT, coordinating with primary care and partners to deliver a nine-month intervention aimed at stabilising health.
