The Trailblazer Resilience Hub in York and North Yorkshire, hosted by Nimbuscare in partnership with Humber and North Yorkshire ICB, transforms support for people out of work due to ill health. We provide timely, compassionate, and trauma‑informed psychological support through a virtual, single‑point‑of‑access service, helping individuals move towards recovery and sustainable employment.
We are now seeking a motivated, empathetic, and highly organised Resilience Hub Care Coordinator to join our dedicated team. This pivotal role lies at the heart of the service, combining care coordination, case management, and clinical administrative support to ensure safe, effective, and person‑centred delivery. The Coordinator will act as a trusted point of contact for patients and partners, supporting individuals through their journey and contributing to service performance, data insight, and continuous improvement. This is an opportunity to be part of a collaborative, purpose‑driven team, making a real impact on recovery, wellbeing, and return to meaningful work.
