We are seeking an experienced and proactive Deputy Nurse Manager to join our multidisciplinary team within the Community Frailty Hub. The postholder will contribute to the day-to-day management, assist in driving quality improvements, and promote best practice in frailty prevention and community-based care. Working as part of a multidisciplinary team, the Deputy Nurse Manager will help ensure that service delivery aligns with organisational, clinical, and regulatory standards while supporting the integration of new services, pathways, and digital innovations.

Main duties of the job:

  • Support frail people to remain independent for longer by providing evidence-based Comprehensive Geriatrics Assessments (CGAs) in the community responding cohesively alongside the wider MDT.
  • Regular attendance at the Integrated Neighbourhood team MDT, supporting the smooth running of these meetings as Chair when required.
  • Work cohesively with the wider MDT, including Palliative and EOL care teams in the community, to coordinate holistic, person-centred care and ensure smooth transitions across pathways.
  • Assist in monitoring and auditing clinical practice to ensure adherence to patient safety and quality standards.
  • Support the Nurse Manager with the line management of Nursing and HSW staff, including day-to-day oversight, supervision, and delegation of tasks.
  • Foster a positive, compassionate, and supportive culture that encourages teamwork, psychological safety, and professional growth.
  • Support the identification of service development needs and assist in implementing redesign initiatives to enhance patient outcomes and operational efficiency.
  • Participate in clinical audits and quality improvement activities.
  • Support local investigations of incidents and events, ensuring learning is identified and actions are implemented.
  • Contribute to the development and review of risk assessments aligned with organisational policies.
  • Promote patient involvement in care decisions and respond to patient and family concerns in a supportive and timely manner.
  • Support staff learning needs and assist in delivering training or signposting to development opportunities.
  • Help ensure all new staff receive appropriate induction and mandatory training.

Person Specification:

  • Significant post-registration experience within community, frailty, acute, or primary care environments.
  • Experience supporting or supervising staff (e.g., mentorship, shift coordination, professional support).
  • Experience contributing to quality improvement, service development, or pathway redesign.
  • Experience working as part of a multidisciplinary team.
  • Experience working alongside palliative care or EOL teams.
  • Registered Nurse (NMC).
  • Evidence of ongoing professional development.
  • Strong clinical assessment and decision-making skills.
  • Excellent communication and interpersonal skills across all levels.
  • Ability to lead, motivate, and support staff, fostering a positive and inclusive team culture.
