Nimbuscare Limited in York is seeking an experienced registered nurse to join the Community Frailty Hub.
You will supervise a team of nurses and health support workers, conducting proactive nurse-led assessments and coordinating care for people living with frailty in the community.
You will work within the Integrated Neighbourhood Teams to deliver a 9-month intervention aimed at stabilising health and reducing crisis, with emphasis on partnership across primary care and community services.
