We work as a multidisciplinary team supporting the individual needs of the patients and those who care for them.
We are seeking an experienced and compassionate Patient Flow Nurse to join our dedicated team. The successful candidate will play a crucial role in facilitating the effective, efficient, and timely admissions and discharge of patients from our hospice care. The Patient Flow Nurse will work collaboratively within the patient flow team, clinical staff, and multidisciplinary teams to assess and address the health and social needs of patients, ensuring continuity of care post-discharge.
Duties:
- Take the lead in ensuring safe and timely admission and discharge of patients, actively identifying and addressing any delays in the discharge process.
- Act as an advocate for patients, carers, and relatives to enhance their discharge experience.
- Facilitate sensitive discussions regarding admission and discharge plans, addressing any disagreements and ensuring all parties are informed and aligned.
- Chair multidisciplinary admission and discharge meetings, ensuring that all relevant stakeholders are engaged in the discharge planning process.
- Collaborate with the clinical staff to develop and implement realistic and up-to-date admission and discharge plans that are centered around the individual needs of each patient.
- Monitor and amend plans as necessary to minimize potential delays.
- Cover for the Patient Flow Lead in their absence, which includes triaging admissions for the inpatient unit, managing advice calls, and organizing respite admissions.
- Maintain concise and up-to-date records of admission and discharge plans and patient interactions using relevant, paper and electronic systems.
- Provide guidance and education to ward staff regarding current legislation, admission and discharge planning best practices, and adherence to NICE guidelines.
- Participate in the collection and presentation of data related to patient admission and discharge and support the development and maintenance of admission and discharge documentation.
