We are looking for an exceptional Outpatient Lead to join our dedicated multi-disciplinary team providing high standards of holistic care to our patients and their families under the direction of the Clinical Director to our patients living with life limiting conditions. You will work as part of the team to provide holistic person-centred care, integrating nursing care with psychological, physical, rehabilitative and spiritual needs. You need to be able to demonstrate evidence-based practice, up to date clinical skills, flexibility and a commitment to the delivery of high quality, effective and efficient nursing care. You will have experience of managing a department including staff and programme management.

Main duties of the job
The Outpatients Lead will oversee the day to day operation, coordination, and development of the Wellbeing and Outpatient Service. This includes providing high quality clinical interventions within scope, leading the wellbeing team, and ensuring a safe, efficient, and compassionate service for patients and their families. The postholder will work collaboratively with the multidisciplinary team (MDT) and external partners to enhance service quality, experience, and accessibility.
Key Responsibilities:
- Oversee the daily operations of the outpatients and wellbeing service, ensuring high standards of care and a positive patient experience.
- Support service planning, pathway development, and improvements in patient flow.
- Liaise closely with the Patient Flow Team and clinical staff to ensure safe and efficient operations.
- Support the inpatient unit during periods of need to ensure continuity of care.
- Conduct clinical observations, assessments, and monitoring aligned with hospice procedures and within professional boundaries.
- Undertake holistic assessments contributing to care planning, including physical, emotional, social, and spiritual needs.
- Support the wellbeing and rehabilitation programme, contributing to group and one-to-one patient interventions.
- Escalate any clinical concerns promptly to the appropriate medical or senior nursing staff.
- Contribute to triage, patient flow management, and safe decision making within the outpatient service.

Key Responsibilities:
- Leadership and Service Coordination: Lead and coordinate the Wellbeing Team.
- Clinical Duties (Within Scope of Practice): Undertake venepuncture, cannulation, and administration of IV therapies safely and competently.
- Patient and Family Support: Support palliative patients and their loved ones with sensitivity, compassion, and respect.
- Multidisciplinary Teamwork: Maintain strong working relationships with the nursing team, medical staff, and wider MDT.
- Governance, Safety & Quality Improvement: Maintain accurate and timely clinical records, including discharge summaries, outpatient notes, and communication with GPs when appropriate.
- Professional Development: Engage in ongoing training, mandatory updates, and clinical supervision.
Person Specification:
- Qualifications: Nursing (RGN/RN Adult), Leadership or management training, Evidence of continuing professional development (CPD), Other relevant wellbeing or psychosocial care qualification, V300 prescriber.
- Experience: Experience of leading or coordinating a service, programme, Experience of multidisciplinary team MDT working, Experience of supporting patients and families with psychosocial, emotional, or wellbeing needs, Experience of assessing needs, planning interventions, and evaluating outcomes using a person centred approach, Experience of supervising, supporting, or mentoring staff, students, or volunteers, Experience of working within governance frameworks, Experience and competence in IV therapy, including cannulation, venepuncture and blood sampling, Previous experience in a hospice or specialist palliative care setting, Experience of service development or service improvement, Experience of managing change within a clinical or wellbeing service, Experience of partnership working with community services, primary care, or voluntary sector organisations.
