JaxCareConnect is a collaboration created by the local nonprofit safety net of free and charitable clinics to assist uninsured adult Duval County residents in finding primary care and a medical home. In addition to establishing primary care, we evaluate the whole person and explore other barriers to healthcare to support the best possible health outcomes. JaxCareConnect was created in collaboration with Agape Community Health Center, Community Health Outreach, Muslim American Social Services, Mission House, Sulzbacher, Volunteers in Medicine, and We Care Jacksonville.

The primary focus of the position will be providing the JaxCareConnect team with outreach and community health education. Additional priorities include administering Social Determinants of Health screenings, community resource education, managing client evaluation calls, and leading the JaxCareConnect Advisory Council.

Major Duties and Responsibilities:

  1. Engaging in outreach and promoting health education topics and healthcare services.
  2. Implementing and executing the program using established protocols.
  3. Managing program evaluations.
  4. Planning and preparing classes.
  5. Engaging in program planning.
  6. Managing recruitment, planning, and implementation for the Advisory Council.
