Symphony Healthcare Services is currently recruiting for a Clinical Pharmacist to join the team at South Petherton Medical Centre and Martock Surgery. You will provide Pharmacy services, which will include supporting, promoting and implementing high quality, evidence-based, cost-effective and safe use of medicines. This role also includes patient medication reviews, helping the practice achieve prescribing targets and supporting the implementation of national and local guidance in order to improve patient care. The salary for this post will be between £53,523 and £58,465 per annum, (pro rata for part time hours). Starting salaries will be offered based on experience. The hours available for this post are 22.5 per week, working pattern/days will be discussed at interview stage.
Main duties of the job:
- Provide sessional clinical support to Martock and South Petherton Surgeries as an independent pharmacist prescriber, seeing patients and leading on medicines optimisation.
- Support the surgery to improve medicines reconciliation on discharge, formulary compliance, repeat prescribing management and medication reviews.
- Support Symphony and Somerset ICB plans to reduce medicines waste in primary care.
- Interpret and provide complex medicines information to patients, carers and healthcare professionals.
- Provide information, advice and guidance on the safe, rational, effective and economical use of medicines.

Person Specification:
Qualifications:
- GPhC registration as a pharmacist
- Certificate or MSc/Diploma Pharmacy Practice (or equivalent experience)
- Completion (or exemption) of CPPE Primary Care Pharmacy Education pathway
Experience:
- At least 2 years of primary care experience
- Clinical Pharmacy experience
- Knowledge and understanding of Primary Care
- Previous experience of working within integrated healthcare organisations or teams
- EMIS - knowledge of system
