An opportunity has arisen with Moray Health & Social Care Partnership for an NHS Health Care Support Worker to join our Community Response Team. Providing end of life care under the guidance and support of qualified staff, along with assisting patients in the community who have had a fall and other occasional support as required., You will have the opportunity to work closely with other health and social care team members in the community, which could include Community Hospitals and our District Nurse Teams. You will also be required to use your discretion and initiative to manage your workload on a daily basis, taking into account any day-to-day potential unexpected changes to need arising from direct patient/client contact.

Although this post is based in the Keith & Speyside locality, you will need to be flexible to cover occasional sickness and holidays in other parts of Moray if required.

You should have or be willing to work towards a Level 3 SVQ or equivalent and have previous experience in a hospital/community setting. Most importantly, we are looking for people with compassion, empathy and good communication skills, together with the ability to provide End-of-Life care to patients in their own home in a dignified and professional manner. This post is over 7-days per week on a rota basis. A full driving licence and access to a car is essential, although a car will be available, you will on occasion be asked to use your own car.
