The predicted salary is between 36000 - 60000 £ per year.

The Lead Nurse will have a key role in leading the multi-disciplinary assessments, case managing and commissioning packages of care for children and young people eligible for CYPCC funding. The Nurse assessor works in partnership with children, young people and their representatives, and other professionals including health, education, and social care. The Lead Nurse will also be supporting the expanding the team and contributing to the achievement of the key performance indicators for the ICB by acting as the first point of contact for children and young people whilst always maintaining patient confidentially., This role will be responsible for:

Completing assessments for Children and Young people with challenging behaviours and emotional needs to determine eligibility for CYPCC funding.

Coordinating and completing MDT assessments and reviews in accordance with the National Framework.

Providing evidence based holistic assessments within the agreed timeframes.

Supporting children and young people with the offer of a personal health budget to meet the assessed needs and outcomes.

Case management for children and young people eligible for funding via CYPCC.

Record keeping as per Leeds continuing care policy.

Working in partnership with children, young people, and their representatives

Building and maintain relationships with Local Authorities, providers, and hospital trust.

Contributing to the ICB’s achievement of their statutory duties.

Supporting the transition of young people to adulthood
