Job Purpose: To assist the Practice Nursing Team by providing high quality care to patients within a primary and community care setting in accordance with CQC regulation, Practice Protocols and Policies. To assist the Practice Nursing Team to ensure maintenance of sufficient clinical stock, hygiene and adherence to control of infection standards. To assist the Practice Nursing Team with the required administration support to ensure effective and efficient re-call systems and reporting. Organisation: Reports to: Practice Nurse Team Leader Responsible to: Practice Manager You will be supervised by members of the Nursing, Medical and Operational Management team, working within the remit of your job description and capabilities at all times. Performance of clinical duties will be dependent on the candidates aptitude and are strictly at the discretion of the Practice. Dimensions: No direct budgetary responsibility, although effective use of resources and stock control with an awareness of cost

implications is required. Training will be provided on Practice Protocols and Procedures, and a Mentor identified. Knowledge, Skills and Experience Required Please refer to person specification, which identifies essential and desirable criteria. Literate and Numerate Venepuncture Skills and Experience Previous Health Care Assistant &/or Phlebotomy Experience NVQ III in Health Care or equivalent Computer Literate Effective communication and interpersonal skills Ability to work as part of a team and independently Ability to organise, prioritise, and manage time effectively Ability to work under pressure. Reliable and confidential Own transport Ability to work flexible hours, with prior arrangement, desirable. Key duties and responsibilities To work as part of the Practice Nursing Team providing high quality care to patients within a Primary and Community Care setting. To administer the 24hr BP and ECG clinics, including integration of the results into the patient clinical record and

maintenance of the equipment. To undertake phlebotomy and anti-coagulation venous sampling within a primary and community care setting. To assist and chaperone GPs and Practice Nurses during clinical procedures and minor/community surgery operations. To carry out assessments, as requested by Practice Nurses in line with Practice Protocols e.g. testing urine, blood pressure checks, heights, weights and taking simple medical history. Report all matters pertaining to patients care changes in condition/circumstances to a relevant member of the nursing or medical team as a matter of priority and ensure that a document record is kept. To ensure treatment rooms are kept in hygienic, tidy and well-organised condition, and instruments and equipment are maintained to a high standard, reporting any concerns to Practice Nurse Team Leader. To assist with Health Promotion displays and audits as required. To assist with the required administrative duties relating to recall systems,

reporting and compliance. To use the Surgery Clinical Computer System in accordance with practice procedures to record consultation details and collection patient data. To maintain accurate and detailed records in accordance with Practice Policies. All contacts with patients must be recorded. You must act at all times with regard to the confidentiality and sensitivity of issues to do with patients, their care and treatment. To report complaints, comments and significant incidents from patients, visitors and staff; to the Practice Manager in compliance with practice policy. To maintain patient confidentiality, in compliance with the Data Protection Act and practice policy. To participate in training, education and learning events organised internally and externally, as appropriate to the post to ensure continuing development. To comply with the practice Health and Safety and Control of Infection Policy and Procedures and ensure that a safe environment is maintained

for patients, visitors and staff. To attend multi-disciplinary meetings, staff forums, practice nurse forums, TARGET sessions and other meetings as necessary. To undertake any other appropriate duties as required by the Surgery Communication and Working Relationships To liaise and work closely with: All members of the Primary Healthcare Team Staff in other areas of Primary and Secondary Healthcare Patients and visitors Other Statutory and Voluntary Agencies Note: This Job Description forms part of the Contract of Employment.
