Overview

We have an exciting opportunity to join the Integrated Heart Failure Service at Liverpool University Hospital Foundation NHS Trust based on the Royal Site. The service operates 7 days per week and requires a highly motivated individual with excellent interpersonal and communication skills. The successful candidate will be responsible for the development of a cohesive management programme for patients with heart failure in line with local and national guidelines. The service is delivered by a proactive, dynamic multidisciplinary Heart Failure Team.

Essential elements of the role include assessing, planning and implementing holistic packages of care for patients and their carers who have a diagnosis of heart failure. The role includes autonomous non-medical prescriber responsibilities, with timely optimization of evidence-based treatment and therapies. The successful candidate will need to demonstrate strong clinical reasoning and prioritisation skills and be able to work effectively both independently and as a member of a team.

Responsibilities

  • Perform specialist assessment of patients’ care needs, plan, implement and evaluate care delivery using advanced clinical knowledge and clinical skills.
  • Lead clinical care by managing a patient caseload, providing expert assessment, planning and evaluation.
  • Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements.
  • Establish and maintain effective communication with patients and carers/relatives.
  • Support patients and their families and ensure that they receive the information to enable self-management and participation in their care delivery.
  • Recognise changes in patients’ condition that require intervention by others and refer on as appropriate.
  • Support, assist and encourage the multidisciplinary team in resolving patient problems by providing new and innovative models of case management.
  • Lead the specialist contribution to the service in the delivery of high standards of patient care; work in partnership with lead clinicians, General Managers and Matron to ensure delivery of high standards of patient care.
