We are seeking a Band 6 CPN for our growing service within the enhanced community dementia team.

The service is currently experiencing growth and transformation, providing locality-based specialist dementia care for pre, peri, and post-diagnostic support.

This role offers a challenging and rewarding opportunity within a multidisciplinary dementia team, with the chance to shape this developing service.

We expect you to have well-developed clinical assessment skills, leadership abilities, and the capacity to develop care plans collaboratively with service users and carers. The main priorities include facilitating rapid diagnosis access, treatment, crisis prevention, intervention, avoiding hospital admissions, and supporting timely discharges.

The service comprises three teams with mental health nurses, occupational therapists, social workers, senior healthcare support workers, and Alzheimer Scotland post-diagnostic link workers. These teams are integral to the Argyll and Bute Enhanced Community Dementia service and collaborate with Community Health Services, Mental Health, Primary Care, and other agencies, including social care.
