Job summary Hertfordshire Community NHS Trust (HCT) is looking for a valued member into their Inpatient Rehabilitation Team. This role provides both clinical and administrative support within the Herts & Essex bed-based team, carrying a personal caseload under the guidance of qualified allied healthcare professionals to deliver high-quality, multi-professional therapy packages. To remain attentive to each patients needs during illness or disability, actively promoting and encouraging their independence, restoration of function, and self-management to support them in achieving their personalised rehabilitation goals. Main duties of the job In this role, you will work as part of the inpatient therapy team at Herts & Essex Hospital, a 32-bedded unit in Bishops Stortford, Hertfordshire, specialising in the rehabilitation of patients with a range of conditions including falls, orthopaedic injuries, general medical issues, and amputations, with most patients following a 17-day rehabilitation pathway. You will work closely alongside Physiotherapists, Occupational Therapists, and Rehabilitation Assistants, as well as the wider multidisciplinary team, to deliver comprehensive rehabilitation packages. Your responsibilities will include supporting qualified staff in delivering both individual and group therapy interventions to promote patient recovery and independence. Career Progression access to Learning & Development opportunities, so that you can take on new roles and challenges Work-life Balance flexible shifts, committed shifts, wellbeing resources and build paid annual leave Opportunity & Access over 50 partner NHS Trusts to give you the flexibility of choice to work how and where you want Job Responsibilities * To assist qualified allied health professionals, including occupational therapists and physiotherapists, in delivering rehabilitation interventions to individuals or groups across various settings, including patients homes. * To undertake routine assessments of patients with acute conditions under guidance, formulate and deliver standard individual treatment programmes, and teach the use of walking aids, exercise equipment, and adaptations safely.emissions testing as required, following BSA and departmental standards. * To utilise assistive devices and technologies appropriately for assessment, treatment, and evaluation, and carry out routine wheelchair assessments and referrals under the direction of qualified staff. * To conduct access visits to service users homes, gather and communicate relevant information, accompany patients on home visits prior to discharge, and contribute to discharge planning with the multidisciplinary team to ensure safe and effective discharge. * To order necessary equipment identified during assessments under guidance, initiate and process referrals to voluntary sector organisations and support groups to aid patients ongoing care. * To perform regular safety checks of equipment, maintain cleanliness and hygiene in line with infection control protocols, follow health and safety policies, report accidents promptly, and comply with organisational procedures, contributing to departmental reviews and updates as needed.
