Senior Community Nurse role at NHS North East London. This post is suitable for a band 5 or an experienced band 6 nurse with a passion for high quality care and service development.

Responsibilities

  • Manage your own clinical caseload of community patients.
  • Support the HCA/rehabilitation assistants with training, supervision, and joint sessions.
  • Work as part of the Rehabilitation and Frailty Team (RAFT) providing daily input for up to 21 days in the patient’s own home.
  • Service hours are 8:00–20:00 with a care element from 7:00–22:00, seven days a week, 365 days a year; you will work occasional weekends and bank holidays on a rota basis.
  • Lead nursing care for a designated caseload; assess, manage, and deliver care to meet the complex needs for patients requiring intermediate and primary care services.
  • Be professionally and legally accountable for all aspects of care; work autonomously and as part of a team; support peers and pre-registration students; liaise with other health and social care professionals and agencies.
  • There is an expectation of seven days working within this role due to service provision.

Requirements / Qualifications

  • Registered Nurse (NMC RN12) with significant intermediate and community care experience at Band 5/6.
  • Ability to lead nursing care for a designated caseload; professional and legal accountability for all aspects of care.
  • Strong communication, collaboration, and initiative; able to work on own initiative and as part of a team.
  • Car driver is essential.

  • High Cost Area Supplement: 5% of basic salary (minimum £1,303; maximum £2,198).
  • Probationary period applies (internal applicants may be exempt under certain conditions).
  • Induction: comprehensive induction up to two weeks for new starters, with mandatory training, systems training, and equipment allocation; induction location at the head office in Rainham, Essex.
