1x fixed-term post (12 months), 37 hours per week, shift pattern – 7 days

A 12 month fixed contract post for a Senior Rehabilitation Support Worker has become available within the Rapid Assessment Link team within the East Dunbartonshire Community Rehabilitation Team. The service provides an integrated assessment and response function to prevent avoidable hospital admissions, facilitate timely discharge, provide rehabilitation to intermediate care beds and provide a period of rehabilitation within the community.

The rehab support worker will carry out baseline assessments, using an agreed framework and present the findings to the multidisciplinary team to enable decision making with regard to future team interventions. They will implement treatment programmes for patients and provide advice to patients and their carers on a range of patient conditions, forms of therapy providing empathy and reassurance. This role also supports the rehabilitation within the intermediate care beds in Westerton Care Home and wider community.

The post holder may also be delegated appropriate tasks by professionally qualified team members including Physiotherapist, Dietician, Occupational Therapist, Speech and Language Therapist, Nurse, and Team Leader. Training, supervision and support will be provided to enable the post holder to work within their agreed competence levels. You should have attained an SVQ in Health and Social Care or equivalent and have good communication skills. Physio and OT rehab support worker competencies are desirable. A current Driving Licence is required for this post.
