An exciting opportunity has arisen for enthusiastic and motivated individuals to join our established Macmillan One to One team as Band 4 link workers.

Working under the leadership of the Macmillan One to One Team Lead, you will provide a person-centred service that enables people affected by cancer to navigate the health, social care and wider systems to access holistic support. You will do this by empowering and promoting the self-management needed for people to live well during and after cancer treatment by proactively identifying the needs of people affected by cancer using Holistic Needs Assessment (HNA) tool.

The successful candidate will have a genuine interest in working with people affected by cancer and have excellent interpersonal skills with the proven ability build and maintain relationships based on trust and respect, recognising people as experts in their own lives.

For this role you will have an HND (or equivalent) qualification in a relevant area such as Health and Social Care or Community Development. Additionally, you should have excellent understanding of social care services relevant to the role, and a good understanding of the overall health and social care environment. There will be a requirement to travel across the Forth Valley area to provide the service in local communities as well as making home visits.

Ability and willingness to travel throughout the area of service delivery is essential for this post.
