An exciting opportunity has arisen for an Advanced Nurse Practitioner to be part of the new Hospital at Home team in Inverness. Our Hospital at Home programme is designed to deliver comprehensive, patient‑centred care, reducing the need for hospital admissions and enhancing patient outcomes. This service will provide acute, hospital‑level care to people in their homes, conditions that would otherwise require acute hospital inpatient care. Delivery of Hospital at Home will enable better person‑centred care experience, which avoids the risks of healthcare‑acquired infection and/or long stays in hospital. Local delivery of a Hospital at Home service fits with the Scottish Government drive to enhance community services and prevent hospital admissions. Funding has been secured specifically to create this service for Inverness.
The specialist practitioners will be supported by Senior Decision Makers and acute Consultants. They will work as part of the Integrated Team, being supported by AHP, Social Work and Care at Home colleagues. This Team forms part of the Intermediate Care options that are available to people in Inverness.
Job Description
We are seeking a compassionate and skilled Nurse to join our multidisciplinary team. In this role, you will provide acute care to patients in their homes, working closely with doctors, specialists, and other healthcare professionals to ensure the highest standards of care.
Key Responsibilities
- Deliver hospital‑level nursing care to patients in their homes.
- Administer medications, perform diagnostic tests, and manage medical equipment.
- Collaborate with a multidisciplinary team to develop and implement individualised care plans.
- Monitor patient progress and adjust care plans as necessary.
- Educate patients and their families on managing their health conditions at home.
- Maintain accurate and timely patient records.
Qualifications
- Registered Nurse (RN) with a valid nursing licence in Scotland.
- Experience in acute care, emergency care, or home health nursing.
- Strong clinical skills and the ability to work independently.
- Excellent communication and interpersonal skills.
- Ability to travel to patients\’ homes within the designated area.
- Compassionate, patient‑focused approach to care.
