NHFT is seeking an experienced Specialist Palliative Care Clinical Nurse Specialist to provide expert advice and clinical support through the SPOA and Urgent Community Response service in Northampton. You will work with patients, families, GP practices and care partners to manage complex end-of-life needs at home, aiming to prevent unnecessary hospital admissions.
The role is part-time (22.5 hours per week), shifts 12pm-8pm, with a pro rata salary of £49,387 to £56,515 per year.
