Responsibilities
- Operate as a specialist pharmacist across renal services within the Trust.
- Support the safe and effective use of medicines to improve patient outcomes and patient safety.
- Lead medication cost-saving and medicines optimisation projects within the renal care group.
- Provide specialist pharmaceutical support for haemodialysis and peritoneal dialysis patients.
- Contribute to medication review and clinical decision-making in renal outpatient services.
- Supervise and support pharmacists working within the renal pharmacy team.
- Support service development and quality improvement initiatives.
- Deputise for the Lead Pharmacist for Renal when required at pharmacy and care group level.
- Collaborate closely with consultants, nursing teams, and multidisciplinary renal services.
Requirements
- Registered Pharmacist qualification and eligibility to practise.
- Experience working within hospital pharmacy services.
- Strong clinical pharmacy knowledge and medicines management capability.
