Advanced Nurse/Clinical Practitioner ANP/ACP

16 to 20 hours per week. Monday to Friday. Working days and hours to be agreed. Part Time.

The opportunity has arisen for an experienced professional ANP/ACP to join our friendly Practice serving a population of 5700 patients. You will be an independent prescriber with experience of working in general practice. You will have a patient-focused approach to improving the quality of care and your team will deliver through innovative techniques and technologies. Main duties of the job:

• The Advanced Nurse/Clinical Practitioner (ANP/ACP) will act autonomously within their professional scope of practice providing care for patients presenting with LTCs, minor and acute illness and injury, working with the GPs to support demand and acute care needs of patients including the review of patients with LTCs. This will be mainly via face to face and to include video, telephone and e-consultations.
• The postholder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
• The Advanced Nurse/Clinical Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
• The postholder will use skills, knowledge and competencies as a qualified Advanced Nurse/Clinical Practitioner in order to be responsible and accountable for managing patient caseloads for treatments, referral, admission and discharging patients as appropriate.

Job responsibilities:

Person Specification:

• Ability to provide quality care.
• Good interpersonal and communication skills.
• Registered healthcare professional with NMC Registered Nurse - a Master's level qualification in Advanced Nurse/Clinical Practice and evidence of working at advanced practice level.
• Independent prescriber with current prescribing qualification and active use in practice.
• Demonstrable experience of assessing and managing undifferentiated presentations, managing risk and uncertainty.
• Ability and willingness to undertake home visits.
• Excellent communication and interpersonal skills, with a patient-centred approach.
• Competent IT skills and ability to work with electronic clinical systems.
• Experience working in general practice or primary care networks.
• Post-registration experience in urgent/primary care, community or acute settings.
• Experience supervising or supporting other clinicians (e.g. first contact practitioners, trainees).
• Involvement in audit, QI projects or service developments.
