About the Team

ASCP is a multi-disciplinary, responsive service. We visit individuals in their own homes, understand their circumstances to provide practical solutions – from equipment to short-term support – helping people regain skills and confidence.

You’ll work alongside an incredible team of professionals, including:

  • Social Workers and Social Care Assessment Officers
  • Physiotherapists
  • Falls Prevention Coordinators
  • Moving & Handling Specialists
  • Reablement Practitioners
  • Health colleagues

Together, we deliver rapid interventions that prevent unnecessary hospital admissions and help people stay independent for longer.

About the Role

  • Support people to maintain independence and avoid unnecessary admissions
  • Contribute clinical expertise to a team tackling falls, frailty, long-term conditions and unexpected crisis
  • Ensure timely, strengths-based interventionsThis is a front-facing role where your decisions matter. You’ll make an impact from the very first conversation, shaping care pathways and helping people live safely and confidently in their own homes.

  • Experienced in Local Authority work
