New Cross Alliance, a three-practice Primary Care Network serving Ilford, is seeking a Clinical Pharmacist with prescriber status to join full-time across the PCN. The role involves medicines management, SMRs, and proactive pharmaceutical care alongside GPs, nurses and other staff.
You will lead medicines optimisation, support chronic disease management, polypharmacy, and deprescribing where appropriate, and contribute to improving patient outcomes.
