We are seeking a Family Support Worker (Grade 4) to join our team at St Thomas Centre, providing early help, practical support and coordinated interventions to improve outcomes for children, young people and their families.
The Role
- Acting as Lead Professional for an allocated caseload, coordinating multi‑agency support plans.
- Using common assessments to identify needs and develop effective, outcome‑focused plans.
- Delivering evidence‑based interventions to promote positive change and safeguard children.
- Engaging families through persistent outreach and promoting their participation in assessments and plans.
- Maintaining high‑quality case recording and contributing to performance and audit processes.
