We're currently seeking a part-time Field Registered Nurse to support clients throughout Binghamton/Syracuse Region in Central New York.
This role offers the opportunity to build meaningful relationships with clients while providing clinical oversight and care in the comfort of their homes.

What You'll Do
- Provides medical oversight for clients in their homes- including but not limited to assessments, obtaining MD orders, monthly contacts, and maintaining an appropriate Plan of Care .
- Communicate with referrers, discharge planners, and physicians to ensure seamless patient care.
- Develop, implement, and monitor individualized treatment plans, ensuring proper care coordination and medication management.
- Perform in-home patient assessments, reassessments, and provide intermittent nursing services.
- Ensure accurate documentation and compliance with agency policies, including patient rights and discharge planning.
- Supervise and provide in-service education for staff, conducting competency evaluations.
- Participate in quality improvement programs and policy reviews.
- Provide 24-hour on-call nursing coverage on a rotating basis.
- May assist with PCA Training Program as Needed.
- ***Sign on Bonus up to $7,500***

Requirements
What You Bring
- NYS licensed and currently registered as a Registered Professional Nurse.
- Home Care experience preferred.
- Thorough knowledge of the scope of practice of professional and paraprofessional healthcare.
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
- Ability to speak effectively before customers/patients or employees of the agency.
- Excellent organizational writing, communication and interpersonal skills.
- Ability to apply common sense understanding to carry out instructions furnished in written, verbal or diagram form.
- Required to participate in on-call rotation (+$7/hour)
