We are currently looking to appoint a Support Planner, who will work under the general direction and supervision of the Business Support Planner Lead as part of the enablement and peripatetic service to ensure efficient day‑to‑day delivery of reablement, long‑term care, extra care, nightcare and day services.

You will:

  • Effectively handle queries and complaints
  • Communicate orally and in writing
  • Keep accurate records and provide management information
  • Use and maintain information technology systems, prioritise workloads
  • Work organisationally and with team members

Knowledge:

  • Issues relating to older people and physical disabilities
  • Health and safety issues in this area
  • Confidentiality

Daily tasks/duties and responsibilities:

  • Allocate work through the reablement IT Management Systems to Home Care Staff to maximise effectiveness and efficiency
  • Plan and ensure continuity of reablement, coordinate staff rotas and work schedules, cover annual leave, sickness and training
  • Make decisions about changes to service delivery with outreach practitioners, managers, communicate with service users, staff and managers
  • Ensure safe working practices by compliance with health and safety legislation and guidance
  • Monitor and inform the reablement coordinator, through the electronic management systems, of changes to actual service delivered against the service user’s individual service delivery plan
  • Input changes to service delivery plans
  • Action care plan changes and inform or discuss with all affected parties, confirm letters, document discussions

Essential requirements:

  • English and maths equivalent to GCSE pass level
  • CLAIT, IBT Level II or equivalent experience

Desirable but not essential:

  • NVQ 3 in customer care
