About the role:

As a Commissioning Broker, youwillbe at the heart of connecting people with the care they need. Acting as the link between families, professionals, and providers, youwill help secure high-quality care packages and placements that truly make a difference.

In this role, you will:

  • Support safe andtimelyhospital discharges, working closely with partners to ensure everything is in place
  • Manage adiverse and sometimes complex caseload
  • Maintainaccurateand up-to-date recordsacross systems
  • Handlesensitive information with professionalism and confidentiality
  • Monitor provider performance tomaintainhigh standardsof care
  • Work collaboratively with managers to improve processes and outcomes
