We are seeking an experienced Practice Nurse to join our friendly and well-established clinical team. You will play a key role in delivering high-quality, patient-centred nursing care and supporting the management of long-term conditions and health promotion across the practice population.
Responsibilities:
- Deliver evidence-based nursing care in line with NICE guidelines
- Manage chronic disease clinics, including respiratory conditions (asthma/COPD)
- Provide wound care, including compression therapy and Doppler assessments
- Undertake cervical cytology, immunisations, and health checks
- Diagnose and manage acute and chronic conditions within scope of practice
- Prescribe under PGDs and review patient medications
- Support Infection Prevention & Control processes, audits, and compliance
- Maintain accurate clinical records and ensure effective clinical coding
- Support safeguarding of adults and children
- Assist with minor surgery when required
- Liaise with external services to support vulnerable and housebound patients
Qualifications:
- Registered Nurse with current NMC registration
- Previous experience working as a Practice Nurse or in Primary Care
- Competence in immunisations, cervical screening and chronic disease management
- Strong communication and organisational skills
- Ability to work independently and as part of a multidisciplinary team
- Confident IT skills and experience using clinical systems
- Understanding of confidentiality, safeguarding and professional governance
- Qualification or experience in asthma and/or COPD management
- Wound care and compression therapy experience
- Non-medical prescribing qualification (or working towards)
- Experience supporting Infection Prevention & Control initiatives
