Journey Hospice Central PA is looking for an experienced Hospice RN for On-Call Coverage 7 on / 7 off.

The Hospice RN will oversee a primary caseload of Hospice patients and provide skilled professional nursing care to them, their families, and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, New Jersey Hospice licensure laws, accrediting body standards, and agency policies and procedures. Responsibilities include the initial assessment, comprehensive nursing assessments, identifying patient problems, and collaborating to develop the Hospice Interdisciplinary Team (IDT) plan of care. The RN implements, reevaluates, and revises the plan of care to meet the patient\’s nursing needs, providing appropriate, comprehensive, and responsive goals and interventions. The RN also teaches, supervises, and counsels patients, family members, and staff regarding nursing care and patient needs, including other related problems at home. The RN works closely with other IDT members to ensure all patient and family care needs are met. Provides supervision to Hospice Aides/Homemakers.

Education/Training/Certifications:

Graduate of an accredited registered nursing program.

Bachelor’s degree in Nursing preferred.

Certified Hospice and Palliative Nurse (CHPN) certification preferred.

PA RN Licensure by the PA State Board of Nursing in good standing.

Valid driver’s license and reliable transportation insured according to PA state requirements.

Skills and Knowledge:

Must maintain a professional attitude and demeanor, communicate effectively, and relate courteously and cooperatively with patients, caregivers, colleagues, supervisors, co-workers, and others.

Must demonstrate optimism, enthusiasm, and a willingness to work constructively with other healthcare team members, showing concern for the well-being of patients and staff.

Interact positively with all staff, supporting the mission and vision of LSMNJ.

Experience:

Minimum of one (1) year of general nursing experience preferred, with current hospice, home health, medical, surgical, or critical care experience.

Experience with pain and symptom management interventions is preferred.

Ability to work within an interdisciplinary setting.

Understanding of hospice philosophy and principles of death/dying.
