Care Coordinator Full-time | Permanent About the Role We are looking for an organised, compassionate and proactive Care Coordinator to join our care team.
This is a key role in ensuring our clients receive safe, reliable and person-centred care in their own homes, while supporting our care staff with effective rotas, clear communication and day-to-day guidance.
Key Responsibilities Act as a main point of contact for clients, families, care staff and professionals.
Match care workers to clients based on care needs, skills, availability and preferences.
Support the Registered Manager with quality monitoring, reviews, spot checks and audits.
Ensure care records, visit notes and office systems are accurate and kept up to date.
Escalate safeguarding, risk, complaints or quality concerns in line with company procedures.
Support recruitment, induction, shadowing and ongoing supervision of care staff where required.
Chairing team meetings and incident responses About You Previous experience in a care coordination, senior care or complex and domiciliary care role is essential.
NVQ Level 3 in Health and Social Care, or equivalent, is desirable.
Strong understanding of person-centred care, safeguarding and confidentiality.
Excellent organisational skills with the ability to prioritise and work under pressure.
Confident communication skills, both written and verbal.
Good IT skills and experience using care planning or rostering systems is an advantage.
A caring, professional and calm approach when dealing with clients, families and colleagues.
A full UK driving licence and access to a vehicle may be required for community-based duties.

Previous experience in a care coordination, senior care, complex care or domiciliary care role is essential, with proven ability to organise care delivery, respond to changing priorities and support care staff effectively.
Strong working knowledge of safeguarding, confidentiality, dignity, person-centred care, infection prevention, medication awareness, risk management and health and safety responsibilities within domiciliary care.
Knowledge of CQC expectations, care planning, risk assessments, MAR charts, rota management and quality monitoring processes.
Excellent organisational, communication and problem-solving skills, with the ability to prioritise a busy workload, manage rota changes, remain calm under pressure and make timely, appropriate decisions.
Experience using electronic care planning, rostering or scheduling systems.
Professional, reliable and accountable, with a caring approach and the confidence to build positive relationships with clients, families, care workers, office colleagues and external professionals.
Flexible, proactive and confident supporting care staff in the community.
Ability to work 37.5 hours per week, take part in the on-call rota and respond appropriately to urgent operational needs.
