New City College is seeking a Transitions Coordinator to support students with SEND, EHCPs and other high-needs learners moving from school into further education. You will be the main point of contact for students, families, schools and local authorities, delivering person-centred advice and building strong partnerships.
You will work with curriculum and support teams to ensure appropriate adjustments, coordinate visits and enrolment, and monitor EHCP processes to meet statutory requirements.
