Job Overview
New Barn Close Surgery is a dispensing GP surgery in Broadway, Worcestershire, serving approx. 8,000 patients. The practice is looking for an experienced Practice Nurse with Diabetes training to work 4 days per week (hours negotiable). The nurse will manage their own clinics, perform routine nursing tasks, and provide diabetes and chronic disease management.
Main Duties
- Lead and run routine nursing clinics, including BP checks, NHS health checks, prescribed injections, BMI and lifestyle advice, suture and clip removal.
- Provide Diabetes reviews and chronic disease management for conditions such as Diabetes, Ischaemic Heart Disease, Hypertension, and other long‑term conditions.
- Perform wound care and implement evidence‑based wound‑care protocols; manage minor traumatic wounds within competency.
- Administer childhood and adult immunisations and provide advice on vaccines as per patient group directives.
- Carry out venepuncture, collect pathological and microbiology specimens (blood, swabs, urine), and perform relevant investigations such as cervical cytology, ECG and dopplers.
- Deliver health‑promotion education, guide patients to appropriate services, and support National Screening programmes.
- Assist with and support Quality and Outcomes Framework (QOF) requirements.
- Maintain accurate patient records and administer consultation notes in line with NMC guidelines.
- Ensure infection control, cleanliness of treatment rooms, and safe storage and disposal of medications and vaccines.
- Participate in practice meetings, mandatory training, and home visits when required (e.g., flu & COVID‑19 vaccinations).
- Communicate effectively with clinical team members, patients, and carers, and adapt communication to meet patient needs.
Qualifications
- Registered Nurse with NMC and an active PIN.
- Diabetes chronic disease management qualification.
Experience
- Minimum 2 years post‑registration experience.
- Experience working as a Practice Nurse in a primary care setting.
