We are seeking a compassionate and dedicated Adult Social Worker to join the Community Mental Health Team (CMHT). This is an excellent opportunity to work with adults facing diverse challenges, including mental health issues, physical disabilities, and social difficulties. About the Role As an Adult Social Worker, you will play a vital role in improving the lives of adults in the community. You will assess needs, develop personalised care plans, and coordinate with other professionals to ensure your clients well-being and quality of life. Key Responsibilities Assess Client Needs: Conduct thorough assessments to evaluate clients physical, emotional, and social needs. Develop Care Plans: Create tailored care plans that address individual goals, reviewing and adjusting them as required. Provide Support and Counselling: Offer guidance and emotional support to clients and their families, helping them navigate challenges.
