The Role:
- Deliver short-term, community support that promotes independence and well-being
- Work flexibly across a large geographical area to respond quickly to referrals
- Support individuals with daily living tasks such as mobility, personal care, meal preparation, and reablement-focused goals
- Accurately document and communicate progress, risks, and concerns to the care coordinator
- Monitor and adapt support based on the individual's progress
You must have:
- A full UK driving licence and access to a reliable vehicle
- Prepared to travel throughout mid and east Devon
- Previous experience working as a care assistant or Support Worker
