Your Company: A values-led health and social care provider is seeking a Registered Nurse to strengthen health outcomes and clinical oversight across supported living services. This role focuses on reducing health inequalities, improving access to equitable healthcare, and helping people to live healthier, safer, and more meaningful lives.You will join a supportive clinical governance function, working alongside operational teams and wider health partners to embed best practice, raise confidence in clinical skills, and enhance person-centred support for individuals with complex and fluctuating health needs. Your Role and Responsibilities While in this position your duties may include, but are not limited to: Support the delivery of a health and wellbeing strategy aimed at improving quality of life, reducing health inequalities, and preventing avoidable ill health. Provide specialist clinical oversight to services supporting people with complex and PMLD health needs across the South East. Deliver health promotion initiatives, including workshops, 1:1 sessions, and service-based engagement, empowering people to take greater control of their health. Promote and increase uptake of cancer screening and preventative health services, using accessible and person-centred approaches. Advocate for equitable access to healthcare, supporting the use of reasonable adjustments and accessible health information. Work collaboratively with people supported, families, and local health professionals to ensure consistent, high-quality health support. Provide specialist advice, training, coaching, and competency assessments to care teams supporting individuals with complex health needs. Support teams with delegated healthcare activities, including hands-on guidance with community healthcare procedures. Contribute to safe and effective hospital admission and discharge planning, supporting continuity of care. Support safe medicines management through clinical advice, audit activity, and troubleshooting. Embed evidence-based practice and recognised clinical standards, including approaches aligned to PMLD core and essential standards. Monitor health-related outcomes and contribute to service improvement initiatives. Maintain accurate clinical documentation and contribute to clinical governance reporting. Identify and escalate safeguarding concerns in line with policy and professional standards. What You Will Need to Apply: Please apply only if you meet the following criteria: Qualified Registered Nurse with current and continued NMC registration. Proven experience in nursing within the learning disability field. Direct experience supporting people with profound and multiple learning disabilities (PMLD). Hands-on experience with community healthcare procedures such as gastrostomy/enteral feeding, catheter and stoma care, oral suction, and related interventions. A full UK driving licence and willingness to travel across the South East. Comfortable working independently across multiple services and from home. A genuine people person, aligned with strong values and a person-centred approach to care. Strong communication skills and confidence engaging with multidisciplinary teams.
