We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

Job overview

Join the Integrated Community Response Service (ICRS) as a Band 5 Community Staff Nurse and help patients remain well and safe in their own homes. You will work within a supportive multidisciplinary team focused on preventing unnecessary hospital admissions and enabling patients to return home safely following acute care. We are looking for a forward-thinking, enthusiastic, flexible and innovative nurse with a strong commitment to team working and delivering high-quality, person-centred care. You will work closely with GPs, London Ambulance Service, Social Services and other community services, with excellent opportunities to develop your community nursing skills and continue your professional development.

Main duties of the job

The post holder will assess, plan, implement and evaluate nursing care for patients in their own homes, supporting admission avoidance and safe, timely discharge from hospital. Working as part of a multidisciplinary team, they will deliver high-quality, evidence-based and person-centred care while liaising effectively with GPs, London Ambulance Service, Social Services and other community partners. The role requires excellent communication, organisation and time-management skills, the ability to work independently and use initiative, and a flexible approach to working across a 7-day service. The successful candidate will be reliable, adaptable and enthusiastic, with a strong commitment to teamwork, patient safety and ongoing professional development.

Experience

  • Recent experience of Nursing at Level 5, or evidence of a back to nursing programme
  • 12 months post registration experience in a relevant setting, e.g. palliative care, care of the older person, orthopaedics, community services
  • Experience of working in a community setting
  • Supervision of junior staff and / or students
  • Experience of working as part of a multidisciplinary team

Qualifications and knowledge

  • Level 1 Registered Nurse- with valid registration with the NMC - RN1 ,RNA
  • Ability to converse articulately on current health issues and demonstrate knowledge in: Concepts of Essence of Care Concepts of team working Professional standards including confidentiality and clinical governance Adult Safeguarding Evidence-based clinical practice
  • Evidence of Continuing Professional Development
  • Demonstrate knowledge of health and safety and personal safety issues
  • Experience or recognised qualification in areas such as: Health Promotion Diabetes Care of the older person Continence Palliative care Medical/Surgical Nursing
