Job description
Job responsibilities
Clinical leadership and patient care:
Lead PC24s clinical service and take responsibility for its day-to-day clinical delivery, the staff who provide care and the standard of care patients receive. The post combines Lead ACP practice with departmental leadership, with around 60% of working time in clinical practice and 40% in management.
Practise autonomously as an ACP. Assess and treat patients attending the primary care centre, and make decisions about investigation, treatment, referral or discharge within professional competence and agreed pathways.
Give colleagues senior clinical advice and support. Where a patient needs care beyond PC24s capability, arrange escalation or transfer and make sure the receiving team has the information it needs.
Keep records accurate, obtain consent and assess capacity where needed. Explain the plan to patients and what they should do if their condition changes.
Keep clinical knowledge and professional competence current and bring relevant changes in guidance into the department's practice.
Demonstrate commitment to developing personal research skills.
Clinical staff and their development:
Line manage the departments clinical staff, excluding GPs. This includes ACPs, UCPs, trainee ACPs and Urgent Care Assistants (UCAs).
Support recruitment and onboarding. Work with the Education Team to ensure new staff understand the service and are suitably trained for their role. Mentor clinical staff and provide additional support to trainee ACPs when an educator or clinical mentor is unavailable.
Hold appraisals and performance reviews, give regular feedback and address concerns about practice, conduct, productivity or capability. Be clear with staff about what needs to improve and follow NEMS policies and procedures when further action is needed.
Agree each clinicians scope of practice and supervision arrangements. Identify gaps in skills or confidence and arrange the support needed to address them.
Make time for regular one-to-ones about staff wellbeing, workload and ideas for the service. Organise clinical supervision and chair regular meetings for all departmental staff.
Set out the clinical staffing and skill mix the service needs. Work with the Operational Service Manager and Rota Team on cover and escalate any risk to safe delivery. The Operational Service Manager coordinates the rota and leads sickness and annual leave processes.
GPs are not currently part of the PC24 team. If asked by the Medical Director, Deputy Medical Director or Medical Lead, support GPs in the service. Formal GP performance management remains with the Medical Lead.
Quality, safety and patient experience:
Take responsibility for safety and quality within PC24. Keep local infection prevention and control arrangements in order, oversee clinical stock and act on safeguarding concerns.
Make sure the department is ready for CQC inspection, with current evidence, sound local arrangements and staff who understand their responsibilities.
Work with the Clinical Governance Lead and Governance Team on incidents, complaints and patient feedback. Provide clinical information and help the department act on learning. The Governance Team coordinates investigations; the HoCD makes sure local actions are completed.
Gather and review patient and carer feedback in good time. Work with senior clinical staff to understand what it says about care at PC24 and make changes where needed.
Involve patients and carers when services are reviewed or developed. Speak up for an individual patient when their concern needs to be heard.
Attend the monthly Patient Safety Steering Group and support the Governance Team in presenting information about PC24. Contribute to clinical audits when required and use the findings to improve care.
Service delivery and development:
Keep a close view of how the clinical service is working. Review access, waiting times, clinical productivity and quality, and follow up when the service is not meeting its standards.
Use clinical and operational information to understand pressures on PC24. Work with the Operational Service Manager and Medical Lead to resolve issues that cross clinical and operational responsibilities.
Maintain an overview of the departments finances. Identify pressures early and explain their effect on staffing, capacity or care. Contribute clinical and workforce information to service planning and business cases.
Keep up to date with developments in urgent care, primary care, integrated care systems and NHS England guidance. Consider what they mean for PC24 and bring forward changes that will improve care or the way the service works.
Lead agreed changes to clinical practice within the department. Contribute the PC24 clinical view to wider service development and help put agreed changes into practice.
Work with local and national healthcare providers to improve patient pathways and develop service models. Explain how NEMS services contribute to care across the wider health system.
Partnerships, guidance and NEMS leadership:
Represent PC24 in local clinical discussions with Sherwood Forest Hospitals, including regular meetings with Emergency Department, paediatric and other specialty teams. Raise matters that need wider medical or organisational decisions through the appropriate NEMS lead.
Attend and present at the weekly Management Meeting. Chair the meeting when the usual chair is absent.
Attend and contribute to the monthly Clinical Performance Group, and chair it when required.
Attend Senior Leadership Team meetings on a rolling rota, representing the departments local leaders.
Lead the drafting and review of departmental SOPs and policies. Arrange medical review and sign-off, and submit them for executive approval where required. Develop clinical guidelines with the Medical Lead, who provides final clinical sign-off.
