You will deliver person-centred, goal-focused rehabilitation in people’s homes as part of an Intermediate Care service, supporting adults with physical and psychological needs to regain independence.
Join a multidisciplinary team, with induction and mandatory training provided. You will work both in the community and with the MDT, including lone working when necessary.
