The post holder will be expected to act as an expert practitioner/role model, delivering the Significant 7 Training Package to support Care Home Staff to develop and maintain clinical practice knowledge and skills. The post holder will use toolkits to enhance clinical competence of individuals/teams and improve the provision of the fundamentals of care.

The post holder will also be expected to support their team, department and organisation to achieve the Trust\’s Values in their day-to-day work.

Responsibilities

  • Deliver the Significant 7 Training Package across Care Homes in the Waltham Forest area.
  • Be involved in the training and development of work‑based learning for care home staff.
  • Provide assessment support to the needs of care home staff.
  • Maintain communication networks to support identified care home staff, community nursing staff and hospital multi‑disciplinary teams for work‑based learners.
  • Take opportunities to train and reflect on practice within the teams and alongside other teams to promote holistic care to service users.
  • Work alongside staff in practice (care homes) to support the development of skills and competencies in the workplace.
  • Use the toolkit to support care workers to identify the health needs of residents and deliver an interactive training session delivered by healthcare professionals in the care home setting.
  • Participate in reflective practice with multi‑disciplinary teams.
  • Work in a multi‑disciplinary way to develop processes to support the learning environment.
  • Seek opportunities to engage in research, attend training and share knowledge with the team leading to change of practice.
  • Keep up to date with trust policies, follow guidelines and report accidents/incidents in line with trust policy.
  • Understand and implement change in line with national and local policy.
  • Monitor fairness and consistency in all work alongside others.
  • Seek opportunities to develop the assessment process and documentation through training.
  • Review with colleagues the suitability of existing placements and learning opportunities and ensure that competencies are completed.
  • Provide accurate advice and information to multi‑disciplinary teams whilst the work‑based learner is with them.
  • Escalate any concerns about the academic or clinical work of a member of staff through the appropriate channels.
