Overview

The post holder will be a Registered Nurse ideally with Specialist Practitioner qualification, and significant experience of working within the community setting. They will be required to hold experience and competence in all clinical skills offered in the community. They will be responsible for assessment, management, planning and delivery of care, including being professionally and legally accountable.

Responsibilities

  • Support peers and locality lead and be an effective team member whilst working on their own initiative.
  • Supervise staff members and students.
  • Support the Operational Lead in the investigation and management of clinical incidents, complaints and maintaining safe staffing levels.
  • Provide case management for a small caseload of highly complex adults on the Integrated Care Team (ICT) caseload with complex multi-diagnosis, high use of statutory services and already receiving face to face care from the ICT.
  • Deliver safe care to users of service across the team and remain professionally and legally accountable.
  • Line manage a team of Community Nurses and Health Care Support Workers; supervise and appraise staff members and students.
  • Demonstrate excellent leadership and communication skills and the ability to travel between sites.
