We are seeking experienced Band 6 Community Mental Health Nurses to join our temporary staffing bank on an as-needed basis, supporting our teams and contributing to the Trust's mission of delivering "the best care by the best people." To be eligible, you must be a Registered Mental Health Nurse with at least two years of post-registration experience. You should possess strong written and verbal communication skills, work effectively as part of a team, and have a solid understanding of mental health issues, including experience in supporting individuals experiencing emotional distress. In this role, you will assess, plan, implement, and evaluate all aspects of patient care. You will collaborate with multi-disciplinary teams and external agencies to ensure high-quality care delivery and must demonstrate the ability to engage service users and their families with empathy and compassion. Ability to travel effectively and efficiently throughout all localities and surrounding areas is integral to the post. Applicants must hold a valid UK driving license and have access to a car for work purposes. Reasonable adjustments will be made to accommodate the needs of disabled applicants.
Main duties of the job
- Foster recovery through empowerment of service users and carers by involving the service user and carer in the planning of their care establishing and maintaining consistent supportive relationships, which encourage increased self-determination and responsibility for service users and their care.
- Offer a range of evidence based interventions and risk management approaches enabling the service user and carers to reduce and manage symptoms and high-risk behaviour.
- Be proactive, innovative and creative in delivering care, enabling the facilitation of service user’s recovery.
- Engage service users and carers in relapse prevention and crisis plans, providing intervention that maximizes the service user’s ability to resolve crisis and prevent hospitalization.
- Facilitate early discharge from both inpatient and Home Treatment Team by providing regular, formal reviews.
- Deliver a flexible and responsive service through practice that is dictated by service users/carers needs.
- Work collaboratively and effectively with members of the multi-disciplinary team and in partnership with other agencies.
- Deliver high quality care and provide solutions to service users with severe and enduring mental health problems.
- Provide care within the Recovery Model of practice maintaining service users within their own environment in the community.
