Overview
NELFT NHS Foundation Trust | Waltham Forest Rapid Response Team. The Rapid Response Team is a progressive multidisciplinary team that provides urgent care across the borough to prevent avoidable hospital admissions. The team operates 7 days a week, 24 hours a day, to support and treat suitable patients in the community with a range of escalated health and social care needs.

Responsibilities
- Work collaboratively with health and social care professionals across the borough and Integrated Care System.
- Lead clinical decision making and undertake enhanced holistic assessment of patient and family needs in partnership with the patient and their family/carer.
- Triage referrals from other agencies or disciplines.
- Coordinate intensive and complex nursing care with other multi-disciplinary agencies.
- Carry out joint visits with MDT colleagues to promote holistic, person-centred assessment and care.
- Provide line management supervision to members of the Rapid Response multidisciplinary team.
- Participate in and complete managerial and clinical supervision.
- Contribute towards audits within the service and assist with Datix investigations/Root Cause Analysis/Serious Incidents and implement lessons learnt.
- Support and mentor pre-registration learners from a variety of disciplines.
- Be a driver and have access to a car (driver requirement may be essential depending on posts).
Qualifications
- Experience in management is required.
- Acute care experience is required.
- Proficient in advanced clinical skills with the ability to assess physical and psychosocial needs and develop a comprehensive, concordant plan of care with patients, carers, and the wider MDT as needed.
- Ability to work collaboratively and carry out joint visits with MDT colleagues to promote holistic assessment and care.
- Be capable of working within a rapidly changing environment and contributing to service development.
Additional information
- Probationary period: This post will be subject to a probationary period. Internal applicants are exempt unless otherwise specified.
- Induction: New starters join the Trust on the first Monday of each month for a comprehensive induction (up to two weeks) including mandatory training and equipment allocation. Induction is held at the Trust head office in Rainham, Essex.
- High Cost Area Supplement β London: 15% of basic salary (minimum Β£4,313 and maximum Β£5,436 per annum pro rata).
- Vaccination: COVID-19 vaccination and booster are encouraged for staff.
