Responsibilities

  • Work collaboratively with health and social care professionals across the borough and Integrated Care System.
  • Lead clinical decision making and undertake enhanced holistic assessment of patient/family needs in partnership with the patient and their family/carer.
  • Triage referrals from other agencies or disciplines.
  • Co-ordinate intensive and complex nursing care with other multi-disciplinary agencies.
  • Applicant to have management experience.
  • Acute care experience required.
  • Be proficient in advanced clinical skills and knowledge with the ability to assess the physical and psychosocial needs and develop a comprehensive and concordant plan of care with the patient, their carers and the wider MDT as needed.
  • Work collaboratively and carry out joint visits with MDT colleagues within the team to promote holistic person-centred assessment and approach to care.
  • Participate and complete managerial and clinical supervision.
  • Provide line management supervision to members of the Rapid Response multidisciplinary team.
  • Contribute towards and carry out audits within the service.
  • Conduct and contribute to Datix investigations/Root Cause Analysis/Serious Incidents and to help support any lessons learnt within the service.
  • Support and mentor pre-registration learners from a variety of disciplines.
  • Be a driver and have access to a car.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement – London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of Β£4,313 and a maximum payment of Β£5,436 per annum pro rata).

Rapid Response Team is a progressive and innovative multidisciplinary team aimed at supporting the population of Waltham Forest, providing urgent Multidisciplinary (MDT) response to prevent avoidable hospital admissions. The team works across 7 days a week, 24 hours a day.

The aim is to support and treat suitable patients in the community, with a range of escalated health and social care needs.
