About the role:
We are currently seeking a dedicated, enthusiastic and flexible individual to join our team as a Work and Health Coach within the new Trailblazer Project.
The Trailblazer Programme is an ambitious, multi-agency initiative led by Neath Port Talbot Council to reduce economic inactivity and improve wellbeing across our communities. We\\\βre currently delivering innovative projects in our Opportunity Hubs and outreach locations, working alongside health professionals, training providers, and employers to help people overcome complex barriers to work. From supporting residents with mental health and long-term health conditions, to tackling digital exclusion, housing instability, and low confidence, we\\\βre at the forefront of creating tailored, life-changing solutions that make a genuine difference in people\\\βs lives.

As a Work and Health Coach, you will provide tailored, one-to-one support to individuals facing significant barriers to work, training, and wellbeing. Using a holistic, person-centred approach, you\\\βll help participants set goals, build confidence, develop skills, and access the right services to move towards sustainable employment or improved quality of life. You\\\βll manage a varied caseload, work closely with partner organisations, and be there every step of the way, from initial engagement through to in-work support, ensuring people have the tools and confidence they need to succeed.
Your daily routine will include:
- Meeting participants for one-to-one coaching sessions to review progress and update action plans
- Conducting initial assessments and develop Individual Action Plans to understand skills, strengths, and personal goals
- Liaising with partner services such as health teams, training providers, and employability programmes
- Recording case notes and updating the digital case management system
- Providing in-work support to help participants settle and sustain employment
- Coordinating access to wellbeing, training, or community resources that remove barriers and build confidence
About you:
Do you have:
- Experience delivering one-to-one coaching, mentoring, or wellbeing-focused support
- Proven ability to engage individuals with significant barriers such as poor mental health, disability, or social isolation
- Skilled in building trust and motivating people using strengths-based approaches
- Ability to adapt communication to suit a variety of needs and audiences.
- Organised, with the ability to manage a varied caseload and maintain accurate records
- 4 GCSE\\\βs grades C or above, or a qualification of a comparable level (eg. Intermediate GNVQ Level 2, BTEC First Certificate, NVQ Level 2)
- Level 4 in Advice and Guidance or employability related qualification, or a working towards gaining this qualification

- Social Prescriber or Link Worker
