Neath Port Talbot County Borough Council is seeking a committed social worker to join the Patient Flow Team. You will work with individuals to identify what matters to them, undertake Mental Capacity Assessments, Continuing Health Care Assessments and lead Best Interest Meetings to ensure safe and timely discharge from hospital.
You will collaborate with professionals across the health board, therapies and external providers to deliver a service that puts citizens at the centre of care.
