The role will also involve:

  • Liaising with families and health professionals
  • Attending multi-disciplinary (MDT) meetings
  • Creating care plans and risk assessments
  • Reviewing documentation, including quarterly reviews, to ensure care plans remain accurate and effective

Our service operates seven days a week, offering a programme of activities and therapies in conjunction with health professionals where appropriate. These are tailored to each individual’s level of need and are designed to:

  • Provide valuable respite for families

Essential / Desirable Criteria

  • NVQ Level 2 or 3 in Health and Social Care (or willingness to achieve within two years of commencement)
  • Ability to use IT systems to maintain accurate records (essential)
  • Registration with Social Care Wales (or willingness to apply immediately upon starting)
  • Experience of working with individuals with complex needs (desirable)
  • A caring, flexible and person-centred approach
