Practice Nurse

We are looking for an experienced Practice Nurse who can provide care to our patients, working alongside our experienced nursing team. We are a busy practice with a list size of approximately 13,500 but we also like to enjoy our days at work and therefore a sense of fun is required! We are looking for a nurse who is already up and running with the management of long term conditions and who may be willing to expand their knowledge of further training, if needed.

Main duties of the job:

• The successful applicant will be responsible for the oversight of our patients requiring long term condition reviews in the major disease areas: diabetes, respiratory, CHD, and frailty along with childhood immunisations, smears and other vaccinations.
• We can provide a friendly and supporting working environment, joining our existing team of 4 Practice Nurses, 3 HCA's, and a team of GP's and ANP's.
• Access to the NHS Pension and a private Simply Health plan is available, along with access to further training should this be required.

Job responsibilities:

• To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team.
• Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs.
• Chronic disease management in at least two disease areas (respiratory, diabetes, family planning, CHD, hypertension).
• Urinalysis, ECGs, Venepuncture, Blood pressure measurements, Routine immunisations.
• Requesting basic pathology tests, following agreed clinical protocols with referral to senior nurses or GPs as appropriate.
• Other tasks including chaperoning and assisting patients who are being examined by another clinician.
• Maintain accurate records and attend and participate in any Practice meetings when required.

Person Specification:

• Minimum of 2 years post registration experience in primary care/GP surgery.
• Experience in chronic disease areas and cervical cytology.
• SystmOne Computer experience/knowledge.
• Willingness to train further.
