Role Purpose

As a Homecare Worker, you will provide high-quality, person-centred support to adults, young people and children, including individuals with more complex care needs, in their own homes and within the community. The role focuses on delivering outcome-based care that promotes independence, personal development, social inclusion, and emotional wellbeing, while ensuring the individual’s safety, dignity, and comfort. The post holder will work in line with Care Inspectorate Wales (CIW) regulations, organisational policies, and best practice guidance.

Key Responsibilities

  • Person-Centred Care & Personal Support: Support adults, children, and young people with personal care tasks, including showering, full body washes, bed baths, toileting, incontinence care, stoma and catheter care. Provide palliative care support for adults where appropriate, ensuring comfort and dignity.
  • Moving & Handling / Transfers: Support individuals with safe moving and handling, using a range of techniques and equipment appropriate to their assessed needs. Ensure all transfers and mobility support are carried out safely and in accordance with care plans, risk assessments, and best practice guidance. Use hoists, slings, wheelchairs, and other mobility aids safely and effectively.
  • Community Access & Social Inclusion: Enable individuals to access community-based activities and social opportunities. Support children, young people, and adults to participate in age and ability appropriate leisure and social activities, reducing isolation and promoting inclusion.
  • Safeguarding & Protection: Always uphold safeguarding policies and procedures, ensuring the protection of children, young people, and adults. Recognise and report concerns about abuse, neglect, or exploitation promptly and appropriately. Complete safeguarding training as required by CIW and our organisation.
  • Record-Keeping & Reporting: Keep accurate and timely records of care, support, and outcomes. Report incidents, accidents, or changes in health or behaviour promptly, following organisational policy. Maintain confidentiality in line with GDPR and professional standards.
  • Behaviours & Professional Standards: Apply Positive Behaviour Support (PBS) principles when supporting individuals with behaviours that challenge. Demonstrate a proactive, flexible approach, prioritising the individual’s goals while maintaining safe, high-quality care. Adhere to all relevant policies, procedures, and CIW standards.
  • Flexibility & Travel Requirements: Travel safely to and between service users’ homes and community settings. Be flexible with hours to meet the needs of clients, including evenings, weekends, and short notice cover where necessary.
  • Outcome-Based Support & Planning: Work collaboratively with individuals, families, and professionals to develop and implement personalised care plans. Focus on achieving meaningful outcomes, including independence, skill development, personal growth, and social engagement. Monitor progress, record outcomes, and adapt support to meet changing needs.
