NCIC in Brampton is seeking a Community Hospital Discharge Navigator to support the multidisciplinary team and carers in preparing patients for discharge from hospital to home or community care. This fixed-term, full-time role is Band 4 with a 36-week contract and a 37.5-hour week.
The post covers six sites, with travel expected and shifts 08:00–16:00 Monday to Friday, weekends on rotation and start time negotiable. The base is Brampton.
