An exciting opportunity has arisen for a band 5 Nurse Practitioner to join our acute inpatient team. You will provide a high level of care and support to people with complex care needs on long term placements.

Job responsibilities

• To understand and follow good practice within the framework for the Mental Health Act, Mental Capacity Act and DoLs and Care Programme Approach.
• To maintain accurate records, both written and computerised to a standard that reflects their level of clinical expertise. This may involve the use of specialised psychometric tools to complement the assessment and evaluation of service user care.
• Liaise with community based services and resources such as Focus, housing, Police and voluntary sector organisations to facilitate the delivery of the most appropriate and best possible care for the individual service user, carers and their families.
• Discuss issues surrounding risk management and plan care accordingly advising others regarding management of care with senior practitioners.
• To respond to service user crisis in a positive pro-active manner.
• To follow clear care pathways, including methods to reduce the risk of further crisis developing.
• To work flexibly within a 24 hour rota covering early, late and night shifts according to service user need.
• To provide telephone support to any service users, carers, outside agencies or clinicians who require information, advice or crisis intervention. This will require the Post Holder to make immediate, effective decisions that will have implications for the positive outcomes of the service users mental health.
• Undertake comprehensive CPA mental health assessments then develop and implement care packages in a variety of settings.
• To assist in maintaining safe custody of drugs in accordance with the Trust policies and procedures.
• To cover 7 days a week, 24 hours a day on a rota.

Person Specification

Qualifications

• RMN or awaiting Registration.
• Evidence of current registration.
• Basic IT Skills.
• Evidence of ongoing professional development since qualification.

Knowledge

• Able to respond to service users, carers and significant others in a person centred way.
• Able to maintain a safe environment for service users, carers, staff and others.
• Able to work as a team member and effectively communicate with service users and other professionals.
• Able to assess individuals, formulate, implement and evaluate care plans.
• Able to participate in clinical supervision.
• Demonstrate an understanding of the Mental Health Act, Care Programme Approach, Mental Capacity Act and DoLs.

Experience

• Experience of working with people with mental health problems.
• Experience of working with older adults with mental health problems.

£32,073 to £39,043 a year Rates as of April 2026.
